Job description: Job DetailsJob Location: Spicewood APC – Austin, TXPosition Type: 40 HRS – 1 FTESalary Range: UndisclosedJob Shift: M-F 8A-5PCENTRAL TEXAS PALLIATIVE CARE ASSOCIATESJOB DESCRIPTIONJOB TITLE: Palliative Care LVNDEPARTMENT: Austin Palliative CareREPORTS TO: Palliative Care Medical DirectorPOSITION SUMMARYThe Palliative Care Team Licensed Vocational Nurse (LVN) will support a team of clinicians in delivering comprehensive palliative care services and provide direct nursing care for patients as needed under the supervision of the Palliative Care Registered Nurse and Palliative Care Medical Director. This individual will be responsible for providing all telephonic and office support required by the team of clinicians. This will include a range of clinical and administrative tasks. Responsibilities will be highly varied, and appropriate candidates will be multitalented, flexible problem solvers who are eager to tackle complex problems. Key roles include:

• Directly providing support to patients, families, and palliative care team with strong communication skills
• Triaging clinical needs during routine office hours for palliative care patients
• Addressing pharmaceutical management of palliative care interventions with patient education, communication with pharmacies and prior authorizations
• Pre-screening referrals for appropriateness for APC program
• Assisting with care coordination by sending and receiving clinical communications with other offices
• As directed, provides residential/home nursing visits to supplement /enhance periodic provider visits.
• Assisting with administrative operations including patient referral intake and scheduling

ESSENTIAL DUTIES AND RESPONSIBILITES

• Assess, identify, and close clinical and non-clinical gaps in patient care.
• Appropriately triage and escalate routine, urgent and emergent patient calls.
• Initiate health plan authorizations and orders labs, radiology and home health/DME and other critical services. Verifying timely delivery of these services and coordinate follow up.
• Assist with managing medications, including identifying potential medication issues, answering patient questions on side effects and medication conflicts, and coordinating refills and prior authorizations. Communicating refill requests to the providers and ensuring timely follow through.
• Identify community medical resources and assist Austin Palliative Care patients and families in accessing those resources.
• Participate in weekly interdisciplinary team meetings.
• Perform nursing assessments in the home/facility as needed.
• Act as a liaison between patients and physicians, hospital staff, home health staff, and other care team members to facilitate ongoing clinical care coordination.
• Help follow up on patients that are in acute care settings and refer to hospice as is appropriate
• Document in a timely manner patient assessments, patient contacts, physician orders and progress notes.
• Assist with clinical data collection for program monitoring
• Assist with patient referral intake and scheduling
• Other related duties as deemed necessary.

MINIMUM TRAINING, EDUCATION AND EXPERIENCE REQUIREMENTSEducation: Associate Degree in NursingExperience: Minimum of 2 years clinical experience required,Acute care experience preferredLicensures & Certifications:Valid Texas LVN LicenseValid driver’s license and proof of automobile insurance
